EFFECTIVENESS OF GENERAL ANAESTHESIA COMPARED TO COMBINED EPIDURAL AND GENERAL ANAESTHESIA ON POSTOPERATIVE OUTCOMES IN GASTRIC CANCER SURGERY

Abstract

Background: This researchers assessed the survival of patients after gastric cancer surgery with general anaesthesia alone against epidural combined with general anaesthesia (EGA) (GA). Methods: 596 gastric cancer patients who were slated for radical resection were retrospectively examined. To lessen selection bias, propensity score matching was done between GA (n=97) and EGA (n=97) at a 1:1 ratio. To find variables that were significantly linked with metastasis, recurrence, and/or prognosis, univariate and multivariate analyses were used. The rates of patients' 5-year overall survival after administering GA alone versus EGA were analyzed. Results: There were 97 participants who underwent EGA and 97 patients that underwent GA after the propensity scores were harmonized. According on multivariate analysis, complications, pN stage, and reconstruction type were strongly connected with prognosis for the total group. By using multivariate analysis, it was determined that lymphadenectomy and pN stage were independent prognostic variables for patients who experienced a relapse and/or metastasis. Conclusions: In conclusion, patients with gastric cancer may profit from EGA because it has greater analgesic and anti- inflammatory benefits, fewer postoperative problems, is more safe, and has a reduced rate of metastasis and relapse, which is helpful for postoperative recovery.
